How to File a Complaint
Upon receipt of this complaint form, a preliminary review will be conducted to determine if the Arizona Department of Financial Institutions (AZDFI) has jurisdiction over the matter forming the basis for the complaint. If the matter is not in AZDFI's jurisdiction, the complainant will be notified. If it is within AZDFI's jurisdiction, the agency will notify the Respondent (appraiser, AMC, property tax agent) to allow them to reply to the complaint and submit their appraisal (if applicable) with their work file. Upon receipt, the Department will evaluate the complaint to determine whether sufficient evidence of a violation of Arizona state statutes, rules, or the Uniform Standards of Professional Appraisal Practice (USPAP) exists to pursue disciplinary action. If additional information is necessary, a staff member may contact you. Further, if the Respondent wishes to contest the Department's determination or sanctions, they may do so by requesting an informal settlement conference, or elect to have a hearing before an Administrative Law Judge of the Office of Administrative Hearings (OAH). Compensation of any monetary losses experienced by Complainants does not fall within the Department's jurisdiction. Unless filed anonymously, you as the Complainant, will be notified when the case has closed.
